Identification

IdentificationYou should attend in person at the Board's office with your original Statutory Declaration, National Police Certificate and correct identification.  An application will be accepted by mail in circumstances where you reside over 100kms from the Melbourne CBD. A certified copy of your identification must be attached to the original statutory declaration and police report.

Acceptable forms of identification must be Australian issued. Two forms of ID are required. One must include a photo and address as stated on your application and come from the following list:

  • Passport (valid for ID purposes for up to two years post expiry as long as a new passport has not been issued)
  • Drivers Licence
  • Immicard

The other must come from the following list:

  • Birth Certificate
  • Change of Name Certificate
  • Citizenship Certificate
  • Marriage Certificate
  • Medicare Card (is not valid if the expiration is less than two months)
  • Visa

For applicants who have never resided in Australia, ID from the following list is acceptable, must one include a photo and address as stated on your application:  Two forms of ID are required.
